The idea that Amy would kill herself and Rory's baby because she doesn't want the reality without him to be real killed this story stone dead for me.

Why? Because for me (YMMV) it was exactly the sort of thing she would do.
And I found it rather moving.
Although, of course, the van wasn't going nearly fast enough to kill both its occupants when it crashed into the house. Then again, it was the Dream Lord's reality, so perhaps he took Amy's intent to kill herself as the prompt to end the game.

She loves Rory, and she truly believes that the world she's in is real (otherwise she wouldn't be so grief-stricken). The last thing she would want to do is kill his baby.

I agree that she wouldn't (in the real world) kill herself and the baby, especially given that the last thing Rory said was 'Look after our baby'. It's interesting and I'm not certain what was intended. Someone can correct me if I've misunderstood but it seemed that, given the Dream Lord was from the Doctor's mind, that it was the Doctor subconsciously controlling both the dreams while Amy and Rory were simply experiencing them. As soon as the village dream ended and they 'woke up' in the Tardis the Doctor set out to blow it up as he knew it was a second dream. Had he realised the whole setup while in the village dream and then deliberately had Amy dream the death ending in order to end the first dream? Amy's Choice remains interesting on each viewing. If the dreams came from the Doctor's mind, then the village one seems to be his idea of what Rory would consider an idyllic life - Amy married to him and about to have their baby, a beautifully done out nursery, Amy baking quantities of cakes, Rory a doctor much loved by his patients, one of whom thinks the Doctor is his junior, even the Upper Leadworth bit - and then does he choose that Amy is devastated by Rory's death in the dream and so jolt her into realising her feelings in real life? Maybe I'm going too far there...
